Does water ever actually flow over Malham Cove?

Yes, but only during exceptional rainfall—it might happen once every few years. When it does occur, water flows over the cliff edge for a brief period, usually when the Watlowes valley area floods and temporary surface flow reaches the top of the cove.

Why doesn't Malham Cove have a waterfall anymore?

The limestone became too porous over time. After the ice age, fissures and bedding planes opened up in the rock, making it easier for water to sink underground rather than flow over the cliff edge. The cove itself was carved when water flowed more freely during and after glacial periods.

Is the walk to the top of Malham Cove worth it?

Yes, it's one of the better short hikes in the Dales. At the top you'll find extensive limestone pavement with distinctive clints and grykes, which show how water has systematically dissolved the rock—the same process that created the underground drainage below.
